In 20 years, the International AIDS Conference has ballooned from a gathering of some 2,000 scientists at the inaugural 1985 meeting in Atlanta, to this year's multi-disciplinary gathering of 12,000 scientists, activists, and people living with the virus. The 2014 conference in Melbourne, Australia covered a seemingly endless array of topics, from hard science to social issues.

At this year's International AIDS Conference, the serious discussions about science and technology were accompanied by a celebratory atmosphere.

The 12,000 delegates here have a lot to feel good about. In just two decades, the disease has gone from a death sentence to a lifestyle. The number of new HIV infections worldwide is the lowest it has been in a decade, as is the number of AIDS-related deaths.

That's been accomplished by two, complementary revolutions: a scientific sprint that has brought together some of the world's best minds to find better treatment options -- and a social transformation that has changed the way we talk about important issues like sex, gender issues, inequality, violence, and education.

Medical advances discussed at this year's conference include improvements in testing and treatment -- and, a pill that may help prevent HIV transmission. That technology, called Pre-exposure Prophylaxis, or PrEP, dominated scientific discussions on the heels of a Lancet study that found up to 75 percent effectiveness in tests of a pill called Truvada.

Dr. James Rooney of Gilead Sciences, Truvada's maker, explains how PrEP works.

"So PrEP is actually a new strategy for preventing HIV infection. It's a strategy that uses HIV drugs that are oftentimes used for treatment, but in this case, the drugs are actually given to individuals who are at high risk for becoming infected."

The event was not without its own tragedy.

Just days before the conference began, six top figures in the fight against AIDS were killed in a plane crash over eastern Ukraine. The tragedy prompted outpourings of grief at the conference, which began with a minute of silence, led by International AIDS Society President Francoise Barre-Sinoussi.

"I would love to be telling you that we were opening this conference in happier times. The extent of the loss of our colleagues and friends is still hard for me to express. We grieve alongside all of those throughout the world who have lost family and friends in this senseless tragedy."

The weeklong event also brought in some big names, like former U.S. President Bill Clinton.

"We are here on the 20th anniversary of the conference to celebrate so much of the progress that has been made because the world made the right decision to fight AIDS and now, to create a generation free of it. We dare not walk away. And all of us should lead the way."

In a way, this tragic epidemic, which has killed an estimated 39 million people since the early 1980s -- and continues to affect about 35 million people living with the virus -- has transformed the world. AIDS has laid bare longstanding inequalities and injustices, and AIDS activists say they can't fight the virus' spread without fighting these underlying issues.

It's a huge price to pay, but one that activists say has paved the path for a more understanding, more interesting, more colorful world.
